General Dentist (full time)

Cincinnati Health Network is a fully integrated care organization providing dental, medical and behavioral health care to all our patients.

Cincinnati Health Network is currently looking for a Full Time General Dentist

This individual will provide quality comprehensive dental care to the community we serve, many of whom are unable to afford or obtain dental care in the private sector.

Essential Duties & Responsibilities

· Examines individuals requesting care, diagnoses their dental/oral conditions, prescribes and carries out, or directs others in carrying out, appropriate dental/oral treatment, or refers individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines.

· Directly effect improvements in patient well-being through accurate and timely diagnosis and delivery of high-quality dental treatment. Ensure the delivery of competent, accurate dental care and treatment to all patients.

· Records patient-dentist transactions as they occur in the patient’s dental record so that the dental record accurately and completely reflects the nature of the contact, the condition of the patient and the care or treatment provided.

· Completes referrals, data collections instruments and other records or paper works as shall be required from time to time by the corporation.

· Ability to manage multiple responsibilities and emergency situations successfully.

· Provide leadership and direction to other dental clinical staff in order to positively effect overall clinical results. Meet or exceed all measurable standards of care, including overall clinical, quality, and productivity goals.

· Develop and ensure favorable relationships with all patients/families. Ensure all patient services are delivered in a caring and professional manner.

Required Education and Experience

· Doctor’s Degree in dentistry (D.D.S. or D.M.D) from an accredited college or university.

· Licensed to practice dentistry in the State of Ohio. Current DEA license.

· Completion of all applicable immunizations.

· Current CPR certification.

· Completion of an accredited general practice residency program is preferred, but not required.

· Previous professional dentistry experience preferred but not required.

Behavioral Health Consultant


Under the direct supervision of the Director of Behavioral Health, the Behavioral Health Consultant is a critical part of the primary care team in our Health Care for the Homeless Program. The Behavioral Health Consultant identifies, triages, diagnoses, and treats patients with complex medical or behavioral health problems by implementing brief and holistic interventions. Utilizing a variety of evidence-based practices, the Behavioral Health Consultant will provide skills training through psychoeducation, education on chronic disease management, stress management, and behavior plans, all to help patients achieve their own wellness goals.

Responsibilities:

·       Actively participate as a member of the integrated primary care and behavioral health team, working alongside physicians, psychiatrists, nurse practitioners, nurses, behavioral health counselors, and medical assistants.

·       Provide behavioral health screening and assessments to patients experiencing homelessness at the McMicken Integrated Care Clinic and other sites.

·       Conduct in-depth assessments for behavioral health conditions when pre-screens are positive.

·       Provide counseling and brief intervention activities for behavioral health and substance abuse disorders.

·       Provide brief interventions for chronic disease care such as smoking cessation, diet, exercise, sleep hygiene, stress relief, medication compliance.

·       Participate in Quality Improvement activities.

·       Work alongside physicians and offer behavioral health advice/intervention when needed.

·       Make necessary referrals and discuss cases with clinicians.

·       Enter patient information and notes in EMR.

·       Make referrals to ongoing sources of care for identified problems.

·       Keep appropriate records and gather data for reporting as required.

·       Other responsibilities as needed.

Knowledge and Abilities Required:

·       Ability to work effectively with teams and a diverse array of professionals and agencies.

·       Knowledgeable about the region’s mental health and substance use treatment system and resources.

·       Cultural sensitivity and willingness to work with patients experiencing homelessness and patients with HIV

Position Requirements:

Education: Master’s in Social Work or Clinical Mental Health Counseling

Professional License Requirements
·       Licensed Independent Social Worker (LISW) or Licensed Professional Clinical Counselor (LPCC) in the State of Ohio.
·       Candidates actively in process towards their independent licensure or who have met all requirements for independent licensure pending approval are encouraged to apply.

Experience: Prior experience working with individuals diagnosed with a substance use disorder or mental health disorder
