Healthcare for the Homeless Program Overview

The Cincinnati Health Network is a system of health care and supportive service providers that create a seamless continuum of care for people experiencing homelessness in Greater Cincinnati and Northern Kentucky.   CHN offers a comprehensive program of primary medical and mental health services, substance abuse addictions treatment, respite care, and referrals to dental and other health services. The Cincinnati Health Network, through the Health Care for the Homeless Program, partners with experienced organizations and community providers to address the gaps in service delivery and improve the overall health status of those experiencing homelessness.

As the only Federally Qualified Health Center in this area dedicated exclusively to meeting the complex health care needs of homeless individuals and families and recognized as a Patient Centered Medical Home by the National Committee for Quality Assurance (NCQA), CHN plays a critical role in serving over 10,000 patients annually.

Healthcare for the Homeless Program Services

The Cincinnati Health Network offers Walk-in, Same-day, and Scheduled Appointments; sees patients regardless of their ability to pay; takes Medicaid, Medicare, and other types of insurance and regardless of the patient’s housing status, will provide assistance to those seeking health care services.

Ryan White Part C HIV/AIDS Program Overview

As the grantee, The Cincinnati Health Network manages the Ryan White Part C Program by partnering with community providers to deliver the highest quality services to individuals living with HIV/AIDS. The program facilitates the identification and care of HIV infected individuals by linking outreach, testing, primary care, physician consultation, behavioral health and medical case management services in Southwestern Ohio, Northern Kentucky and Southeastern Indiana.

Ryan White Part C HIV/AIDS (CHN Collaborative Partner) Services/Locations

Caracole, Inc.
Caracole operates a variety of housing opportunities for People Living with HIV (PLWH) from a Shelter Plus Care program to rapid re-housing and permanent supportive housing opportunities. Caracole is also a RW Part B provider, offering medical case management and other supportive services to over 1500 individuals annually. Over the last year, Caracole has taken over the state funded HIV testing program originally operated by Planned Parenthood. Midwest AIDS Training and Education Center (MATEC)

Cincinnati Health Department
Operates 5 Federally Qualified Health Centers throughout the City of Cincinnati and provides Sexually Transmitted Infection screening and limited HIV testing. Patients with positive test results care referred to RW Part C program for care. Call: 513-357-7320https://www.cincinnati-oh.gov/health/
